FACTS

A DBS Disclosure Form, has over 1000 fields that may need to be filled in by you, not including continuation sheets that may be required.
If you make a mistake, go over the lines, forget to fill in a field or tick a box correctly or if any part of your application is not legible to the DBS's computerised form scanners, your application will be delayed and may be rejected.
Rejected applications still incur an application fee.
